Feedback Forms

We wish to thank you for taking time to provide feedback about the Alabama Professional Development Modules. 

• General Instructions
   

• Online Feedback Form (Internet Explorer 4.0 or later, Netscape 6.0 or later, and AOL)

• Online Feedback Form (Netscape 4.0 through 4.76)

• Non-online Feedback Form (in Word for Windows Electronic Form completion format - right click to download). 

• Feedback Form - Print Version (in Word for Windows format suitable for printing and completing manually - right click to download)

General Instructions

The module feedback forms can be completed and submitted online.   All forms are the same.  To save loading time, the online forms are specific to your browser.  The first form is specific to Internet Explorer 4.0 or later browsers and Netscape/AOL 6.0 or later browsers. The second form is specific to Netscape 4.0 through 4.76 browsers.   After completing the forms, click on the submit button to finish. (Note: On dial-up modems, the form may take several seconds to load).

If you have problems with the online forms or you do not want to submit the form online, you may wish to download the Microsoft Word version of the form (right mouse click to download to your computer).  Instructions for returning the completed form are on the form.  The Word for Windows Electronic Form has been formatted using drop down menus, check boxes, and expandable fills so you can complete the form electronically.  The Word for Windows print format is suitable for printing a copy and completing the printed form manually. If the form opens on your PC or MAC as "read-only" select the "no" response to re-save the file or save with a new file name if you chose the "yes" response.

Note:  Questions that require a rating box response are checked for completion when the form is electronically submitted.  If you do get an edit notice, please rate the response indicated for completion.  However, you can override this check should you wish to leave a question blank.
