Although the files can be opened in your browser, it is recommended that you download the files to a directory on your computer hard drive, especially if you do not have a high speed internet connection.  The files can then be opened, printed, and saved for your use.  To download files, right click on the file link and save the file to the hard drive of your computer.

• Teacher Evaluation Record Program (Microsoft Excel Program - right mouse click to download) (posted 12/09/04 as Version 1.2)  This Excel workbook contains the teacher evaluation instruments.  All instruments are linked to minimize redundancy of recording demographics and scores and can be maintained as an electronic evaluation file for a teacher.  The instruments and scores can be printed for hardcopy files. Version 1.2 corrects the limitation of text entry in the script tab forms and allows text entry on all forms to be entered in a text box.
• School Profile of Teachers Program (Microsoft Excel Program - right click to download) The school profile of teachers is an Excel workbook that provides a school summary of all ESRs for teachers.  This summary identifies strengths and areas for improvement for the school as a whole.

  Teacher PDP Form
  Note:  Please read page 62 of the PDP section of the manual.  The top half (planning portion) of both sides of the PDP is completed by the educator and evaluator either at the Evaluation Summary Conference or at another time shortly after a full evaluation/review  is completed. The educator supplies information required of him/her on the lower half of the sheet (assessment results and benefits) after completing the planned activities and assessments. The evaluator provides designation of 7.1 and 7.2 alignments, comments, scores, and next steps to be taken after reviewing the educator’s assessment results and benefits descriptions.
